Non-cardioembolic stroke and transient ischemic attack (TIA) are heterogeneous conditions, some variants of which associated with a high short-term long-term risk cardiovascular events. The article presents clinical portraits patients in very groups: 1) extracranial atherosclerosis severe stenosis/subocclusion/occlusion or CT signs atherosclerotic plaque instability; 2) all intracranial atherosclerosis, especially those symptoms hemodynamic compromise multiple infarcts; 3) mobile/ulcerative atheroma the aortic arch. For timely identification these subtypes, it is necessary to use radiological clues diagnostic search. In addition, following TIA may be at risk: an ABCD2 score ≥4 points ABCD3-I ≥8 points; vertebrobasilar basin, "crescendo" "limb shaking" type, orthostatic TIA, as well warning capsular bridge syndromes. Assigning patient one considered categories requires immediate administration enhanced secondary prevention, will discussed second part this article.
